Assumption of Costs

In order to be able to settle costs via a third party credit card, we need the corresponding authorization from the cardholder. Please only use this form for this purpose.
We also need a legible copy of the front and back of the credit card so that we can check the card details and compare the cardholder's signature with the cost assumption declaration.
We reserve the right to reserve a sum equal to the expected costs on the credit line of the card as security.
